Please read this disclosure carefully before enrolling in D’NuVi Direct Primary Care.
D’NuVi Direct Primary Care is a private-pay primary care membership program.
“This agreement does not provide comprehensive health insurance coverage. It provides only the provision of primary care as specifically described in this agreement.”
DPC membership is not health insurance, major medical insurance, or a health maintenance organization.
Membership covers only the primary care services specifically described in the D’NuVi Direct Primary Care Membership Agreement.
The DPC Membership Agreement, standing alone, does not satisfy the health benefit requirements established under the federal Patient Protection and Affordable Care Act, as amended.
D’NuVi Medical Care strongly encourages DPC Members to obtain and maintain health insurance or other appropriate healthcare coverage for services not provided through the DPC membership.
Insurance may be needed for services including:
Emergency department care;
Ambulance services;
Hospitalization;
Surgery;
Specialist care;
Imaging;
Outside laboratory testing;
Prescription medications;
Pharmacy-administered vaccines;
Durable medical equipment; and
Other healthcare services not included in the DPC Agreement.
DPC membership should not be considered a replacement for comprehensive health insurance.
D’NuVi Medical Care may participate with one or more health insurance plans for services provided outside the Direct Primary Care Membership Agreement.
Participation with an insurance plan does not convert the DPC membership into insurance.
D’NuVi will not bill a health insurance carrier for services covered under the DPC Membership Agreement.
Services provided outside the DPC Membership Agreement may be billed separately when permitted and appropriate under the applicable health plan and provider agreement.
Members may use applicable insurance benefits for eligible outside services, subject to the terms of their individual health plan.
Subject to clinical appropriateness and Practice policies, DPC membership includes:
Establish Care visit;
Routine primary care visits;
Annual preventive/physical examination;
Appropriate acute and sick visits;
Chronic disease management;
Medication management and routine refill management;
Preventive screening review;
Immunization review and pharmacy recommendations/orders;
Laboratory ordering and interpretation;
Imaging orders;
Specialist referrals and care coordination;
Appropriate telehealth visits;
Secure patient portal messaging for nonurgent matters;
Review of relevant outside medical records;
Routine medical forms and letters when appropriate; and
Preventive care-gap review.
Same-day appointments may be available for established DPC Members based on clinical need, appointment availability, and location. Same-day access is not guaranteed.
New patients must complete required intake documents at least 24 hours before their Establish Care visit.
The Member is financially responsible for healthcare services, products, medications, laboratory charges, imaging charges, specialist charges, facility charges, or other expenses not specifically included in the DPC Membership Agreement.
Examples include:
Emergency and hospital services;
Specialists;
Surgery;
Imaging charges;
Outside laboratory charges;
Prescription and compounded medications;
Pharmacy-administered vaccines;
Durable medical equipment;
Pathology;
Outside procedures;
Botox;
IV infusions;
Wellness injections;
Medical Weight Loss medications;
Specialty or elective laboratory testing; and
Other third-party healthcare expenses.
For medically appropriate routine DPC laboratory testing obtained through D’NuVi:
The Member pays the applicable laboratory cost only. D’NuVi does not charge its routine blood-draw or coordination fee.
For elective, specialty, functional, optimization, or patient-requested laboratory testing that is not part of medically indicated routine DPC care:
The Member pays the applicable laboratory cost plus a $25 D’NuVi draw/coordination fee.
Members may elect to use an outside laboratory or applicable insurance benefits when appropriate.
Your specific membership type and monthly membership fee are identified in your signed DPC Membership Agreement and Recurring Payment Authorization.
A one-time $49 nonrefundable enrollment fee applies at enrollment.
The enrollment fee is separate from monthly membership dues and is not credited toward future membership payments.
If you are enrolled as a Founding Member, your Founding Member pricing and applicable duration are identified in your DPC Membership Agreement.
Members select recurring billing on either:
The 1st of each month; or
The 15th of each month.
Requests to change the billing date or make alternate payment arrangements must be received at least three calendar days before the scheduled billing date.
If payment fails:
A five-calendar-day grace period applies.
Payment may be automatically retried.
A one-time $10 late fee may be assessed after the grace period.
Non-emergency DPC membership services may be suspended if payment remains outstanding.
Membership may be terminated after 30 calendar days of nonpayment.
Re-enrollment after termination is subject to the then-current enrollment fee, membership rate, requirements, and membership capacity.
After membership becomes active, cancellation requires 30 days’ written notice.
Members remain responsible for applicable membership fees due during the required notice period.
Before the initial membership effective date, a prospective Member may withdraw without owing a monthly DPC fee for a coverage period that has not begun. The $49 enrollment fee remains nonrefundable.
Active DPC Members currently receive:
15% off qualifying D’NuVi memberships and program fees, including qualifying Medical Weight Loss program fees;
10% off qualifying IV infusions and wellness injections; and
Botox at $10 per unit.
Discounts cannot be stacked with other promotions or discounts unless specifically authorized by D’NuVi.
Outside medication, pharmacy, laboratory, imaging, and other third-party costs are not included in these discounts unless specifically stated.
D’NuVi reviews and documents immunization needs as part of primary care.
Routine vaccines are not stocked or administered by D’NuVi Medical Care.
When appropriate, vaccines will be recommended or ordered through an outside pharmacy or other provider.
Members may use applicable insurance or pharmacy benefits for those services.
Secure portal messaging is for nonurgent matters.
D’NuVi responds to portal messages within one business day.
Business days are Monday through Friday, excluding holidays observed by D’NuVi.
